Provide picture of your art or product on jpeg file for review either via e mail to Michele@ocdesignersource.com or mail to 21520 Yorba Linda Bl. #302 Yorba Linda, CA 92887. We will use the application and picture to review your work and then contact you. If your application has been approved, we will set up an appointment if necessary or we will load your gallery upon completion of the final contract and indemnification agreement which we will supply to you upon the approval of your application. We will contact you within 3 business days after reviewing your application. We will load your gallery for $120.00 with the conditions listed in our final contract which we will thoroughly review with you. Some products will require more time to load than others due to complexity and may be subject to a small upcharge at cost to create the gallery. By completing the information on this form, you are acknowledging that all the information is true to the best of your knowledge, and you have read and understood all the information stated above. We look forward to seeing your work.
